Important health and safety documents for international travel for undergraduates

All students traveling internationally are required to submit an Assumption of Risk and General Release Form to the office sponsoring their travel. Students studying abroad for credit should submit the signed forms, complete with emergency contact information, to the Office of International Programs. Students who are traveling independently may turn in the forms at the OIP as well.

Students traveling to countries under Travel Warnings on the State Department website are asked to complete a different waiver:

Students are also advised to pick up their laminated Harvard emergency contacts, and a copy of the I-SOS card to carry in their wallets while abroad.
