Agassiz Equipment Loan Form

Loans of equipment from Agassiz Theatre must be cleared in advance with one of the Technical Directors, must be accounted for with this form on departure and return, and must be used by and taken care of by persons qualified to operate it.  The show borrowing equipment assumes financial responsibility for loss or damage.  Agassiz does not normally loan lighting or sound equipment.  The person borrowing equipment undertakes to make certain that the installation is safe, and that all people using any equipment are properly trained.

Returns are due on the day after the show closes, unless a Technical Director has authorized an extension.  The show may be billed up to the full replacement cost (purchase or rental) for any equipment returned late, and may be charged late fees until the materials are returned.  Shows which borrow equipment without full permission may be charged the full replacement cost plus any damages deemed appropriate.

Note: To borrow a complete TDCTP Toolkit requires a checkout by one of the TD's so we can inventory the kit before it leaves. Email us (Liz Dean, Tom Morgan) to arrange a time.

Loan Procedure:

1. Clear the equipment loan with Tom or Liz.
2. Fill out a Billing Information Form so we know who to bill in case of problems.
3. Fill out the form below when taking the equipment.
4. Fill out the form below after returning the equipment.
